Abstract

Type 2 Diabetes Mellitus (T2DM) is a growing health crisis in the United States, affecting millions of people, and is associated with significant morbidity, mortality, and financial burden. Clinical Quality Measures (CQMs), include hemoglobin A1C, blood pressure control, statin use, and annual screening for nephropathy and retinopathy, which all effect patient outcomes and organization performance. A primary care office that serves approximately 11,000 patients in Madison County, Alabama (AL) found several CQMs related to T2DM that fell below the recommended Healthcare Effectiveness Data and Information Set (HEDIS) goal. The National Committee for Quality Assurance (NCQA) developed and maintains a set of performance measures called HEDIS, which recommends a goal of 80% in multiple diabetes CQMs. A Diabetes Program was implemented with the goal of improving diabetes CQMs, improving patient outcomes related to T2DM, and provider compliance to ordering appropriate test and performing appropriate screenings related to T2DM. The program evaluation examined the effectiveness of a Diabetes Program. Key findings and recommendations for sustaining the program and program reproduction have been identified.
